The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services of funeral director and staff. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected.

BASIC SERVICES OF FUNERAL DIRECTOR AND STAFF: $2,995.00

This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains.) This fee includes the following:

Personnel available 24 hours a day, 365 days a year to respond to initial call.
Arrangement Conference
Coordinating service plans with cemetery, crematory, and/or other parties involved in the final disposition of the deceased.
Clerical assistance in the completion of various forms and permits associated with a funeral.
Also covers overhead, including facility maintenance, equipment and inventory costs, insurance expenses, and government compliance.

PREPARATION OF THE BODY:

Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial.

USE OF FACILITIES AND STAFF:

Use of Facilities and staff services for up to four hours of visitation outside normal office hours and, if funeral is the next day, up to one hour of visitation prior to the funeral. Our services include set-up of visitation area, placement of encased remains, display of floral arrangements, supervision of attendance during the visitation. $795.00

Use of Facilities and Staff Services for up to three hours on the same day as service during normal office hours on weekdays; our services include set-up of visitation area, placement of encased remains, display of floral arrangements, supervision of attendance during the visitation. $595.00

Additional time for visitation (per hour) $195.00

Use of Facilities and Staff Services for Private Viewing (One hour) $325.00

Use of Facilities and Staff Services for Funeral Service in Our Chapel; our services include coordinating the funeral arrangements, supervision of funeral, and staff to assist with the funeral ceremony. $595.00

Use of Facilities and Staff Services for Memorial Service (without remains present) in Our Chapel; our services include coordinating the memorial service arrangements, supervision of the service, and staff to assist with the service. $595.00

Staff Services and Equipment for Memorial Service (with or without cremated remains present) at Other Facility; our services include coordination of the memorial service, supervision of the service, and staff and equipment to assist with the service. $595.00

Staff Services and Equipment for Graveside Service; our services include accompaniment of remains to cemetery, supervision of graveside service, and staff and equipment to assist with the service. $495.00

Funeral Director supervision only for urn committal or celebration of life at cemetery or other facility with no coordination, equipment, or additional staff. (weekdays only) $195.00

Additional Fee for Funeral, Memorial Service or Visitation on Saturday. $350.00

Additional Fee for Funeral, Memorial Service or Visitation on Sunday or Holiday. $1,000.00

TRANSPORTATION:

Transfer of deceased from place of death to funeral home (within a 25 mile radius). Additional distances will be charged at $3.00 per mile. $325.00

Funeral vehicle (Hearse) (within a 25 mile radius)
Additional distances will be charged at $3.00 per mile. $350.00

Family/Clergy Vehicle (within a 25 mile radius) (up to 4 passengers)
Additional distances will be charged at $3.00 per mile (subject to availability). $150.00

Flower/Service vehicle (within a 25 mile radius)
Additional distances will be charged at $3.00 per mile. $150.00

Transfer to or from airport (Louisville or Indianapolis)
Additional distances will be charged at $3.00 per mile. $400.00

Transfer to and/or from crematory $250.00

Transfer of deceased to or from place of autopsy or organ donation to funeral home (within a 25 mile radius) Additional distances will be charged at $3.00 per mile. $450.00

FORWARDING REMAINS TO ANOTHER FUNERAL HOME: $4,715.00

This charge includes removal of remains (within 25 mile radius), Basic Services of Staff, obtaining necessary authorizations, embalming and use of preparation room, transportation to airport (Louisville or Indianapolis) or other place of shipment (within 25 mile radius). This charge does not include air or other shipping charges, shipping container, visitation, or ceremonies. This charge applies to shipment within the Continental United States, only (Additional distances will be charged $3.00 per mile).

RECEIVING REMAINS FROM ANOTHER FUNERAL HOME: $3,745.00

This charge includes Basic Services of Staff, transportation from airport (Louisville or Indianapolis) or other shipping destination (within 25 mile radius), and transport of remains to cemetery or crematory (within a 25 mile radius). This charge does not include any coordination of service, visitation, or ceremonies.

DIRECT CREMATION: $3,395.00 to $11,190.00

If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are alternative containers and other cremation containers, including containers made of wood, fiberboard, and other composition materials.

We also offer a selection of cremation oriented caskets upon special order.

Our charges for direct cremation (without visitation or ceremony) include removal of remains (within a 25 mile radius), transportation to crematory, return of remains, and Basic Services of Staff.

Direct cremation with container provided by the purchaser (All such containers must meet any applicable crematory requirements). $3,395.00

Direct cremation with Alternative Cardboard Container provided by the funeral home (Such container will be the choice of the funeral home, not suitable for viewing). $3,645.00

Direct cremation with Cloth Covered Casket (Steel doe covering, corrugated construction, ivory twill interior). $5,290.00

A cremation package which includes a visitation period and/or a funeral ceremony with the use of a rental casket unit is outlined on Addendum A of this price list.

IMMEDIATE BURIAL: $4,590.00 to $12,385.00

Our charges for immediate burial (without visitation and any rites or ceremonies at the funeral home, graveside, or elsewhere) includes removal of remains (within a 25 mile radius), handling of unembalmed remains, casketing, dressing and cosmetizing deceased, use of funeral coach for transportation to the cemetery (within a 25 mile radius), service vehicle and basic service of staff. (Additional distances will be charged $3.00 per mile)

Immediate burial with casket provided by the purchaser (All caskets must meet any applicable cemetery regulations). $4,590.00

Immediate burial with casket selected from funeral home (In addition to cost of casket as determined by Casket Price List). $4,590.00

Immediate burial with Kinsey Cloth Covered Casket, steel doe covering, corrugated construction, ivory twill interior. $6,480.00

CASKETS: $1,890.00 to $9,190.00

OUTER BURIAL CONTAINERS: $1,790.00 to $16,790.00
